Dad’s Corner: Smart Ways to Save Money at the ER

By Rob Castellucci – Office Manager at Head2Toe Pediatrics & dad of two rambunctious kiddos

Why I’m Writing This

As a parent, nothing spikes your heart rate (or drains your wallet) faster than an unexpected trip to the emergency room. Thanks to Head2Toe, my family has sidestepped plenty of “better‑safe‑than‑sorry” visits—but accidents still happen. When they do, I lean on a few money‑saving strategies that any parent can use, and I’m sharing them here so you can, too.

1. Start With a Quick Call to Dr. Lastra

  • 24/7 Access Is Your Wallet’s Best Friend. Members can text or call Dr. Lastra first. She’ll let you know whether the issue can be handled at home -  far cheaper than the ER.
  • No Membership? Families without easy physician access often default to the ER “just in case,” racking up big, avoidable bills—yet another reason I’m grateful for Head2Toe!

2. If You’re Already in the ER, Make the Visit Count

  • One‑and‑Done Mindset. Ask the ER team to handle anything that could prevent a second visit (imaging, labs, wound checks, etc.).
  • Built‑In Advocate. Dr. Lastra can speak directly with the ER physician to be sure essential tests are ordered now, not “come back in a few days.” (The ER team has final say, but coordinated care makes a huge difference.) In fact, Dr. Lastra will call ahead to the ER so that the doctor is fully aware of your kiddos situation before you even arrive.

3. Follow Up on Less‑Than‑Great Care

  • Document Everything. If bedside manner was poor or basic guidelines were skipped, jot down names, times, and details while they’re fresh.
  • Report to Patient Relations. Thoughtful feedback helps improve care. If there were legitimate issues with the care you received, it may prompt the hospital to adjust or even reduce your bill.

4. Tackle the Bill—On Your Timeline

  • Don’t Rush to Pay. Medical providers can’t place unpaid bills on your credit report for 365 days after the initial statement, giving you time to dispute charges or work out a payment plan — (Experian).
  • Rules Getting Even Better. A new Consumer Financial Protection Bureau rule finalized on Jan 7 2025 will remove medical debt from lender‑facing credit reports entirely later this year — (CFPB press release).
  • Call in the Pros. Services like Goodbill negotiate on your behalf and only keep 20 % of what they save you (pricing details). Worth every penny if it spares you hours on hold.

5. Play the HSA Long Game

  • Pay With Cash or Credit First. If you have the funds, cover the bill outside your Health Savings Account. You can reimburse yourself later—there’s no time limit as long as you keep the receipt (Fidelity HSA FAQ; IRS Publication 969).
  • Triple‑Tax Advantage Reminder. HSA contributions go in pre‑tax, grow tax‑free, and come out tax‑free for qualified medical expenses. The longer the money stays in the account, the more it can grow. This is why you want to avoid using HSA funds whenever possible.

Key Takeaways

  1. Prevention saves: Quick access to Dr. Lastra avoids many ER trips altogether.
  2. Be strategic in the ER: Get everything handled in a single visit, with Head2Toe backing you up.
  3. Advocate afterward: Thoughtful complaints can improve care and potentially lower costs.
  4. Use time (and Goodbill) to your advantage on billing.
  5. Keep HSA funds invested when possible.
